Consent Form for Debit of Life Insurance premium
Note –
- This is one time request authorized by you for the above policy number and premium for future dues (if any) shall not be debited from your account without your consent
- In case of Unit Linked products, if the premium is debited post 3 PM, NAV would be allocated towards your policy on next working day
- Revival / Renewal of the policy is subject to fulfilment of Underwriting requirements (if any) and decision of HDFC Life would be considered as final
Disclaimer –
Please note that by entering the OTP, you are agreeing to the terms & conditions of such debit authorization
and also expressly authorize HDFC Bank to debit your account towards insurance premium amount plus taxes,
if any for your HDFC Life Policy. The consent so given by you shall be binding on you and that it is post your
consent, the bank is debiting the amount of premium (as mentioned in the said page) and that you shall
unconditionally agree to keep the HDFC Bank indemnified at all times in respect of the debit authorization
given to HDFC Bank on call & confirmation on the aforesaid information by entering the OTP received on your
mobile no registered with the Bank.
